Understanding the Cause

ED can be an early warning sign of cardiovascular disease, diabetes, or other systemic conditions, sometimes appearing years before a cardiac event. That is why a proper evaluation matters beyond just addressing the symptom. Our evaluation includes a thorough medical and sexual history, assessment of contributing factors such as medications, cardiovascular risk, diabetes, hormonal status, and psychological factors, and targeted lab work when indicated.

Oral Medications

First line treatment for most men is an oral PDE5 inhibitor such as sildenafil (Viagra) or tadalafil (Cialis). These medications are safe, effective, and widely available as affordable generics. We prescribe them routinely and help you find the best price. Generic versions have become very affordable at local pharmacies, and we help you find the best option. We do not steer you toward expensive brand names or proprietary formulations, and we do not upcharge on prescriptions or require you to buy through our office.

Penile Injection Therapy

For men who do not respond adequately to oral medications, or who cannot take them due to medical contraindications, penile injection therapy with trimix is an excellent next step. Trimix is a compounded medication containing three vasoactive agents that is injected directly into the side of the penis using a very small needle. The injection produces a reliable, firm erection within 5 to 15 minutes that typically lasts 30 to 60 minutes.

While the concept understandably gives men pause, most patients report that the injection is far less uncomfortable than they expected, and the reliability of the result makes it worthwhile. We teach you the technique in our office and adjust the dose until we find what works.

Penile Prosthesis: The Gold Standard for Refractory ED

For men who do not respond to medications or injections, or who want a permanent, on demand solution, the penile prosthesis is the gold standard. It is the treatment with the highest satisfaction rates in all of sexual medicine, with patient and partner satisfaction consistently exceeding 90 percent.

We implant the Coloplast Titan inflatable penile prosthesis. The device consists of two inflatable cylinders placed inside the erectile bodies of the penis, a small pump in the scrotum, and a fluid reservoir. To achieve an erection, you squeeze the pump. When you are finished, you press a release valve and the device deflates. The entire device is completely concealed inside the body. It is not visible, not detectable by a partner, and does not affect sensation or orgasm.

Dr. Mian is our experienced prosthetic surgeon and performs penile implant procedures at Emplify Health in Green Bay. The surgery typically takes about an hour, and most men go home the same day or the next morning. Recovery takes approximately four to six weeks before the device is activated.
