Prostate Cancer Screening and Risk Assessment

Prostate cancer screening starts with a simple PSA blood test and digital rectal exam, but at Urology Associates, it does not stop there. When a PSA result is elevated or trending upward, we use a layered approach to determine your true risk before jumping to a biopsy.

Our screening and risk assessment tools include PSA testing and PSA trend analysis over time, IsoPSA (an advanced blood test that evaluates the structural forms of PSA protein and provides a more accurate risk assessment than standard PSA alone), prostate MRI to visualize the gland and identify any areas of concern, clinical risk factor evaluation including family history, race, age, and prior biopsy results, and genetic testing performed in our clinic for men with a strong family history or other risk indicators.

This comprehensive approach allows us to avoid unnecessary biopsies in many patients. When the MRI is reassuring, the IsoPSA is favorable, and the overall clinical picture is low risk, we can confidently recommend monitoring rather than an invasive procedure. For patients who do need a biopsy, the MRI guides us to target suspicious areas precisely, improving diagnostic accuracy.

Biopsy and Genomic Testing: Getting the Full Picture

When a biopsy is indicated, we perform MRI targeted prostate biopsy using the UroNav fusion platform, which overlays your MRI images onto real time ultrasound to precisely target suspicious areas. We offer both transrectal and transperineal biopsy approaches, and both are available in our office with office based sedation for your comfort.

Urology Associates was the first practice in the Green Bay area to offer transperineal prostate biopsy. The transperineal approach accesses the prostate through the skin of the perineum rather than through the rectal wall. This is an important distinction because it virtually eliminates the risk of serious post biopsy infection, which is a known complication of the traditional transrectal approach. As antibiotic resistance has become a growing concern, the transperineal technique has emerged as the preferred method at leading institutions.

When cancer is found, we send the tissue for Decipher genomic testing, which analyzes the molecular biology of your specific cancer to assess how aggressive it is and how likely it is to spread. This information, combined with your biopsy results, MRI findings, and PSA data, helps us recommend the most appropriate next step. For many men, that next step is active surveillance rather than immediate treatment.

Active Surveillance: Avoiding Overtreatment

Not every prostate cancer needs to be treated right away. For men with low risk disease, active surveillance is a well established, guideline supported approach that monitors the cancer closely over time rather than proceeding to immediate surgery or radiation.

At Urology Associates, approximately one quarter of our prostate cancer patients are managed with active surveillance. These men undergo regular PSA testing, periodic MRI imaging, and repeat biopsies as needed to ensure the cancer remains stable. If the cancer shows signs of progression, we transition to treatment while the window for cure remains wide open.

The advantage of active surveillance is that it avoids the potential side effects of surgery or radiation, including impacts on urinary, bowel, and sexual function, for men whose cancer may never pose a threat in their lifetime. It takes a disciplined program and an experienced team to do active surveillance well, and we have built that program over many years.

Robotic Prostatectomy: High Volume, Experienced Surgeons

For men who need surgical treatment, we perform robot assisted radical prostatectomy using the da Vinci surgical system at Emplify Health (Bellin) in Green Bay. Our surgeons have performed over 2,000 robotic prostatectomies combined, making us the most experienced prostate cancer surgery team in the region. Surgical volume matters: studies consistently show that higher volume surgeons achieve better outcomes in cancer control, continence recovery, and preservation of sexual function.

The procedure typically takes 2 to 3 hours and includes pelvic lymph node dissection when indicated. When appropriate based on the cancer location and grade, our surgeons perform nerve sparing surgery to maximize the chance of preserving erectile function after recovery. Most patients spend one night in the hospital and go home the following morning.

Radiation, Cryotherapy, and Other Treatment Options

We offer and coordinate the full range of prostate cancer treatments. Robotic prostatectomy using the da Vinci surgical system is performed by our high volume surgeons at Emplify Health (Bellin). We also work closely with independent radiation oncologists and the Emplify system to offer external beam radiation therapy and HDR (high dose rate) brachytherapy. For select patients, we offer cryotherapy (freezing of the prostate) as both a primary treatment and as salvage therapy for recurrent cancer. We are the only practice in the region to provide cryotherapy and have treated several hundred patients. Your treatment recommendation will be individualized based on your cancer grade, stage, anatomy, age, health, and personal priorities through a shared decision making process with your care team.

Advanced and Metastatic Prostate Cancer: Comprehensive Care From Diagnosis Through Every Stage

At Urology Associates, our commitment to your care does not end after initial treatment. We manage the entire spectrum of prostate cancer, including recurrent disease, castrate sensitive metastatic prostate cancer, and castrate resistant metastatic prostate cancer. Our dedicated advanced prostate cancer team coordinates all aspects of medical management from our office, working closely with medical oncology and radiation oncology as needed.

Our advanced disease management includes androgen deprivation therapy (ADT) and advanced androgen receptor blockade, all current generation oral therapies for metastatic prostate cancer, bone health management and monitoring, Provenge (sipuleucel T) immunotherapy (which we are the only practice in the region to offer and have administered to numerous patients), and coordination with radiation oncology for palliative or metastasis directed radiation when appropriate.

Frequently Asked Questions About Prostate Cancer

At what age should I start prostate cancer screening?

Current guidelines recommend that men discuss screening with their doctor starting at age 50 for average risk men, age 45 for men at higher risk (African American men and men with a first degree relative diagnosed with prostate cancer before age 65), and age 40 for men at the highest risk (multiple first degree relatives with early prostate cancer). If cancer recurs after treatment or progresses to metastatic disease, our team manages all phases of care, including androgen deprivation therapy (ADT), advanced androgen blockade, bone health management, and immunotherapy. We are the only practice in the region to offer Provenge (sipuleucel T) therapy and have treated numerous patients with this immunotherapy. We coordinate closely with medical oncology and radiation oncology, but your care remains centered in our office with a dedicated team experienced in both castrate sensitive and castrate resistant metastatic prostate cancer.
