Low testosterone is a clinical diagnosis, meaning it requires both symptoms and laboratory confirmation. We start with a detailed history of your symptoms, medical history, and medications, because many factors beyond testosterone can affect how you feel. We then check a morning total testosterone level, which is the standard screening test. Depending on the clinical picture, we may also check free testosterone, LH, FSH, prolactin, and other labs to help distinguish between primary and secondary causes of deficiency. This matters because the underlying cause can influence which treatment approach is most appropriate.
We treat symptoms, not numbers. If your total testosterone is 280 and you feel fine, you do not need treatment. If your total testosterone is 340 and you are struggling with fatigue, low libido, and brain fog, a conversation about treatment is absolutely warranted. Population reference ranges are averages, not individual thresholds.
Testosterone replacement is not always the first step, and it is never the only step. Meaningful improvements in energy, body composition, mood, and even testosterone levels can often be achieved through lifestyle modifications alone or in combination with therapy.

Topical gels are applied daily and provide steady absorption throughout the day. They are convenient and well tolerated, though they do require attention to skin contact with partners and children until the gel has dried. Injectable testosterone is administered on a regular schedule, and most men learn to self inject at home after an initial teaching session in our office. Testosterone pellets are placed under the skin every three to six months, providing steady release without daily or weekly dosing. Oral testosterone options are also available.
Dosing is not standardized. We start at an appropriate dose based on your levels and symptoms, then adjust based on how you feel and what your follow up labs show. The goal is symptom improvement at the lowest effective dose, not chasing a target number. Follow up labs monitor for side effects including elevated hematocrit, PSA changes, and metabolic parameters.
